Building Innovation in Community Care

The future of SolutionHealth is a system integrated with new and alternative providers, providing care outside the walls of the hospital, relying heavily on our ability to meet our families’ social, emotional, and physical needs in their homes and within their communities.

To help achieve that goal, in March 2022, Home Health & Hospice Care (HHHC) and SolutionHealth signed a combination agreement to combine services to provide enhanced post-acute care services to the people of southern New Hampshire. HHHC is now an official member of the SolutionHealth network, which already includes Southern New Hampshire Health and Elliot Health System who joined to form the entity in 2018.

“We are absolutely pleased to formalize our affiliation with Home Health & Hospital Care,” said Brad Kreick, SolutionHealth CEO. “Together with Home Health & Hospice Care, we will meet patients with the care they need, where they need it. We’ll do this collaboratively, to provide the best of our services for healthier communities.”

This agreement now allows both organizations to work together, inclusive with Elliot Health System’s VNA of Greater Manchester and Southern New Hampshire to develop a fully integrated, regionally coordinated and transformative system of post-acute care for the community.   Integration plans have already begun in an effort to support a seamless transition and build the framework for future success.
