Home Health & Hospice Care (HHHC) and SolutionHealth have signed a combination agreement after recently receiving clearance to proceed by the Charitable Trusts Unit of the NH Department of Justice. HHHC is now an official member of the SolutionHealth network, which already includes Southern New Hampshire Health and Elliot Health System who joined to form the entity in 2018.

We’ll do this collaboratively, to provide the best of our services for healthier communities.”
This agreement now allows both organizations to work together, inclusive with Elliot Health System’s VNA of Greater Manchester and Southern New Hampshire to develop a fully integrated, regionally coordinated and transformative system of post-acute care for the community. Integration plans have already begun in an effort to support a seamless transition and build the framework for future success.
